To help shorten our patients time waiting to be seen we ask that you complete several forms at home and bring them with you to your appointment.

If you are coming for an appointment, you will need to complete the patient history form and patient information and bring them with you to your appointment or complete these forms at the time of your appointment.  If you will be completing these forms in our office we ask you to arrive 15 minutes before your appointment.

Things to bring with you:

      1. Insurance cards
      2. A list of all your current medications and eye drops
      3. Someone to drive you home if you are having a dilated exam

Additional things to bring to your CATARACT EVALUATION: Your oldest pair and current pair of glasses.
